main entrance screen This is the opening screen.  The view emergency medical information button opens a web browser showing all the collected data, except the information stored in the "documents manager"  Those item can be displayed by entering a password.  You should give this password to your emergency contacts.This information is in html format and is shown in a browser window.

Main entry choice screenHere is the main entry choice screen.
Utilities enable backing up data, changing passwords and restoring data.
Verifications show when data was last entered and provides reminders to add pertinent information.
Expirations act as reminders to update inforation such as medications running out, dates of last physical, or the last time you had your eyes checked.

All your general information is entered here,  Note that you can even add a photo (it has to be square or it will distort). Enter as much information as you can as this page is of primary importance.  Be sure to update this page on a regular basis, especially for phone numbers.


Enter your emergency contacts here Be sure to inform them of the password so they can inform first responders if necessary. Note that it is possible to put them in the priority of your choice. 


Tell all about your doctors here. This page has lots of value, if you take the time to fill it out. It contains spaces for all the information an emergency responder could want, or even you, if you have trouble remembering who your internist is and when your last visit was.

Enter your insurance information here. This enables you to input insurance info of every sort.  Make sure you enter supplemental insurance, accidental death, dental, auto insurance, etc.  There are places for policy numbers, expiration dates, and policy dates.

All your medication information goes on this page.This page lets you list everything you take even if it is not a prescription, where it was filled (or bought), and how many refills are available. This page is valuable at the doctors office when they ask what medications you are taking. You can put the physical description and include any other notes you might deem necessary.

These files stored here can be password protected by your Doctor so other medical personnel can read them, but not be changed without his password thus insuring accurate information.  You can store all your records here, including EKGs, EEGs, digital mamograms, Xrays any reports that are part of your history.
